Procedures For Obtaining a Criminals Records Check (Ontario Provincial Police)

The individual must complete a Volunteer/Applicant Screening form Consent to Disclosure of Personal Information. Download the Volunteer Screening form.

The individual must present two pieces of identification (driver's license, health card, valid passport, etc), one of which must contain a photo of the individual, when presenting your Screening Form for processing at the OPP Detachment in Brighton or Cobourg.
